Grade Report Requests

Instructions for Requesting a Grade Report

If you would like to request an Official Grade Report for a term, a request form with your signature is required. Click on the link below to open the grade report request form:

Print out the form using the print function on your PC. Complete the form carefully, be sure to sign it, and submit it to the Registrar’s Office in one of three ways:

  • Deliver to the Registrar’s Office in St. Thomas Hall, room 301
  • Fax to the Registrar’s Office at  (570) 941-4148
  • Mail to the Registrar’s Office at 800 Linden St., Scranton, PA 18510-4684

If you do not need an official copy of your grade report (for example, for employer reimbursement), you may also view your grades on the UIS of My.Scranton and also print them out. You would log into the My.Scranton portal at http://my.scranton.edu and then proceed to Self-Service (UIS).
