Instructions for Students to Request Verification of Enrollment
(Note: If for Veterans education benefits, refer to
Veterans Enrollment Certifications and Information)
To request verification of your enrollment for health insurance or other purposes, an Enrollment Verification Request Form with your signature is required. Click on the link below to open the form:

Print out the form using the print function on your PC. Complete the form, sign it and mail or fax it to the following:

Instructions for 3rd Parties to Request Verification of Enrollment
To request verification of enrollment for a current or former student, contact the Registrar’s Office at (570)941-7721 or email at registrar@scranton.edu. The University of Scranton defines enrollment information as directory information under the federal Family Educational Rights and Privacy Act (FERPA). Under FERPA, students have the right to prevent the release of directory information. If the indicated student or former student has exercised this right and, therefore, has a confidentiality flag on her or his record, the Registrar’s Office will inform you that a signed release from the student or former student will be required to release the information.
